Medicare GP Chronic Condition Management Plans
Under a GP Management Plan (GPMP) or Team Care Arrangement (TCA), patients with a chronic condition such as diabetes may be eligible for up to 5 individual Medicare-subsidised allied health services per calendar year. These services are shared across all eligible professions (dietitian, exercise physiologist, diabetes educator, and podiatrist) and are not a separate allocation per profession.
- A valid referral (GPMP or TCA) from your GP is required to access Medicare bulk billing or subsidised rates.
- You are responsible for payment at the full consultation rate if your Medicare-subsidised services have been exhausted for the calendar year, or if your Medicare claim is unsuccessful for any reason.
- The current Medicare benefit is $63.40 per session for eligible allied health services under a Chronic Disease Management plan.
- You can check how many services you have used via Medicare Online or the MyGov portal.
- For further information, refer to the Medicare article: Allied health services for people with a chronic condition and complex care needs (Services Australia).
Existing Care Plans — Transitional Arrangements to 30 June 2027
If you have an existing GPMP or TCA that was created before recent Medicare policy changes, transitional arrangements allow you to continue accessing subsidised allied health services under that plan until 30 June 2027, provided the plan remains current and your GP has not yet reviewed or replaced it under the new framework. Please speak with your GP to confirm whether your current referral remains valid and how many services you have available.
Cancellation Policy
We understand that circumstances change. However, missed appointments represent time that could have been offered to other patients managing their diabetes care.
- We require at least 24 hours’ notice if you need to cancel or reschedule your appointment.
- A cancellation fee of $39.95 applies to late cancellations (less than 24 hours’ notice) and non-attendance.
- This fee will be charged securely to the credit or debit card provided at the time of booking.
If you are unable to attend and cannot provide 24 hours’ notice due to an emergency or unexpected illness, please contact our reception team as soon as possible on (07) 5306 9500 and we will do our best to assist you.
Why Do I Need to Provide Credit Card Details for a Bulk-Billed Appointment?
We ask all patients—including those attending bulk-billed appointments—to provide credit or debit card details at the time of booking. Your card is not charged simply for making a booking.
Card details are held securely and may only be charged in the following circumstances:
- Missed appointments or late cancellations: If you do not attend or cancel with less than 24 hours’ notice, a $39.95 cancellation fee will be charged to your card.
- Declined or ineligible Medicare claims: If your Medicare claim is declined (for example, because your referral has expired, your Medicare-subsidised services for the year have been exhausted, or you do not hold a current GPMP/TCA), you will be responsible for the full consultation fee. The current Medicare benefit is $63.40 per session, and any remaining balance will be charged to your card.
Providing card details allows us to protect appointment availability for all patients and ensures that our clinicians are fairly compensated for their time. If you have any concerns about this requirement, please speak with our reception team before your appointment.
Referrals & Selecting the Correct Appointment Type
To access Medicare or DVA bulk billing at our clinics, a valid referral from your GP (or treating specialist, for DVA patients) must be in place before your appointment. If the correct referral or appointment type is not in place, you will be responsible for the full consultation fee.
Choosing the Right Appointment
- Initial appointment: Select this if you have never attended our clinic before, or if it has been more than 12 months since your last visit to the same profession. An initial consultation allows your clinician to complete a full assessment and establish your care plan.
- Review appointment: Select this if you have previously attended our clinic within the last 12 months and are returning for ongoing management with the same profession. If you are unsure whether to book an initial or review appointment, please contact us before booking.
- Podiatry appointments: If you have been referred specifically for a high-risk foot assessment (for example, due to peripheral neuropathy or vascular disease associated with diabetes), please indicate this when booking so that sufficient time is allocated.
Selecting the wrong appointment type may result in insufficient time being allocated for your consultation, or in your Medicare claim being processed incorrectly. Our reception team is happy to help you select the right appointment — please call (07) 5306 9500.
Private Health Insurance & Payment
Many private health funds provide benefits for dietetics, exercise physiology, and podiatry services. The level of rebate varies depending on your fund and the level of cover you hold.
- Payment is required at or before the time of your appointment.
- We have HICAPS available at all clinic locations, allowing you to claim your private health rebate on the spot and pay only the gap amount.
- EFTPOS and major credit cards are accepted at all clinics.
- Please contact your health fund before your appointment to confirm your entitlements and any applicable waiting periods.
- If you have questions about payment options or would like to speak with a member of our reception team before attending, please call (07) 5306 9500.
Healthy Lifestyles Australia may update fees, booking terms, and cancellation policies from time to time. Current fees will be confirmed at the time of booking or by contacting our reception team on (07) 5306 9500.
